Pharmaceutical care valuation as a professional and documented service with an influence on pharmacoeconomic therapy aspects at old patient

Introduction. Implementation of pharmaceutical care (PC) in a community pharmacy can minimize the risk of consequences of inappropriate pharmacotherapy. However, for this new service, a suitable documentation should be developed which will gather a detailed information from patient in order to assess the factors determining the effectiveness of pharmacotherapy. Aim. Verification of documentation that will allow to collect relevant data for PC. Matherial and methods. The study was conducted between January 2014 and June 2015 in Jarocin within 54 patients (41.0 % men and 59.0 % women) at age over 65 years who were counselled by family doctor and at least two other professionals. The pharmacist completed PC documentation during patient’s interview in community pharmacy. Results. The study confirmed that 6 patients should be included in PC proces. These individuals with regard to the remaining 48 patients only with pharmacist counseling: applied at least 10 different drugs (p = 0.002) and interrupted drug therapy without physician counsultation (p = 0.0201). In this group patients much more often didn’t abide by the doctor’s recommendations (p = 0.0351). The collected data and future studies may asses PC efficiency in terms of pharmacoeconomic analysis. Conclusions. This study provides new data about PC implementation in Poland. The system of documentation can contribute to professional judgment of patient pharmacotherapy to avoid drug problems and improve the quality of life. Analysis of results should indicate the advisability of PC implementation in community pharmacies to determine the cost of drug reimbursement which are very often improperly used.
